Participant Information
A list of class participants must be submitted to the State MG Program Coordinator in order to complete the background checks. This should include the name to be used on mailings, the address of each individual, phone number and an email address (if they have one). This should be submitted electronically. This information will be entered into the statewide database and will be used for one part of the background checks and sending the newsletter. Because the newsletter is sent electronically by default to all new participants, it is important to request email addresses from participants and forward that to the MG Program Office.
- If at all possible, please provide your records in an Excel file with the following info in separate columns: First name, last name, address, city, state, zip code, phone number and email. You can add any other columns you want, but I need that basic information in separate columns in order to import the data into the statewide database. Otherwise I have to spend lots of time retyping and risk making lots of mistakes in the process. (If you have your records in an Access file, please export these fields into an Excel table.) You can create your own file or use the MG Training Class List Template.

- Try to use standard abbreviations in the address (eg. Dr instead of Drive, Rd, Blvd, Ln, etc.) and avoid using any punctuation.
- Email this file to semahr@wisc.edu.
- There will always be late registrants and sometimes additions/changes in contact information. It is easiest for me if you send a new file with just the additional people. But if you resend a file that you’ve added to, it is VERY helpful to indicate the new parts by using boldface, highlights or some other designation (otherwise I have to spend a lot of time looking through all the records to find the additions or changes). If you only have one or two people to add you can just send their contact info in the body of an email instead of the entire file if you wish.
